Refer a friend or family member to any of our 3 locations and you will be entered into a drawing at the end of the month.

Who is eligible? Current patients, former patients and our Facebook friends.
How do I make a referral?  Simply ask your friend or family member to enter your name as the referral source on their patient information sheet during their initial visit.
How many times can a person be entered?  You will be entered into the drawing for each patient you refer to us between December and February.  However, the referral must be seen at one of our locations before March 1, 2012.
When will the winner be announced?  The first week of March.
When does the contest end? February 29, 2012
